Healthcare Workers Exposures Assessment Questionnaire
This questionnaire is to help Stanford University Principal Investigators and other supervisors in determining if healthcare workers under their supervision (including university-affiliated postdoctoral scholars) need respiratory protection against Airborne Infectious Diseases (AirIDs) and/or need safety training and medical surveillance for other hazards associated with their assigned healthcare activities.
AirIDs
- Healthcare workers determined to need respiratory protection will be included in the Stanford University Respiratory Protection Program per Cal/OSHA (Occupational Safety and Health Administration) requirements [Title 8, CA Code of Regulations, Sections 5144, 5199].
Blood or Other Potentially Infectious Materials (OPIM)
- Healthcare workers determined to have or potentially have contact with blood or other potentially infectious materials (OPIM) will participate in medical surveillance per Cal/OSHA requirements [Title 8, CA Code of Regulations, Section 5193].
Hazardous Drugs
- Healthcare workers determined to have or potentially have contact with hazardous drugs shall receive job-specific training on the hazards of these drugs and how to reduce risk of exposure [Title 8, CA Code of Regulations, Section 5194] as well as participate in medical surveillance as recommended by Federal OSHA.
